The
assurance of quality of care is vital. At SSTI,
we also know that dignity and independence are incredibly
important to elderly loved ones. We are a specialized
resource group that helps families maintain the
highest level of functioning and independence for
elder adults. We draw on years of experience, and
our in-depth knowledge of elder care issues, to
work directly with both the aging adult and their
family members to carefully craft individualized
care plans. We offer a simple 3 step process and
you can begin at no cost to you. Please call us
at any point and we are happy to help walk you through
the process.
Step
1. The Initial Assessment
Our care managers will visit a senior in any environment,
in their home, the hospital, an assisted living
or skilled nursing facility, or our office. We work
with you to complete a comprehensive assessment
of the needs of an elderly adult. We examine the
physical, social, environmental and financial needs
of the elderly individual as well as the needs of
adults facing the responsibility for care of an
aging loved one.
Step 2. Care Planning For The Elderly
Adult
Based on the assessment findings, SSTI works privately
with both seniors and/ or their family to create
an individualized plan of care. The senior and family
are included in the process and have absolute control
over the services they receive. We do not believe
in a one-size fits all care model. We regard each
person and situation individually. SSTI will offer
options, resources, and assistance as desired in
coordinating services.
Step
3. Monitoring
One of the most important ingredients for a successful
plan of care is monitoring to ensure that the goals
are not only met, but sustained. Our team provides
scheduling, supervision, and advocacy for our clients.
If a plan is not working successfully, we will work
with the senior to revise the plan.
